Consent to the Processing of Personal and Sensitive Data

pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (hereinafter the "GDPR") and Act No. 101/2000 Coll., on the Protection of Personal Data (the Czech Personal Data Protection Act, hereinafter the "PDPA").

By submitting information about myself, I hereby grant to Ms Denisa Walterová, doing business as Denisa Walterová – 9. VLNA CASTING, with its registered office at Kodaňská 598/28, 101 00 Prague 10 – Vršovice, Identification No. (IČO): 62569457 (hereinafter "9. VLNA CASTING"), as the data controller, my explicit consent to the processing of my sensitive and personal data for a period of 30 years (including by automated means and, in particular, through collection, storage on data media, disclosure, adaptation or alteration, retrieval, use, transfer, dissemination, publication, retention, exchange, sorting or combining, blocking and destruction) for the following purposes:

• identifying my relevant needs as a collaborator of 9. VLNA CASTING
• performance of a contract and provision of services
• offering and providing suitable offers to conclude a contract for appearing in audiovisual works, or arranging similar work for me
• keeping records of collaborators

The personal data processed on the basis of this consent include: first name, surname, birth surname, academic title, address of permanent or, where applicable, temporary residence, delivery or other contact address, telephone numbers — mobile, home and work, plus an alternative telephone number (the number and name of a close person), other contact details — Skype, Facebook, Instagram and other social networks, personal website, e-mail address, date of birth, identity card number, gender, occupation, possession of a driving licence (and the category of driving entitlement), bank account number, whether the individual is a VAT payer and/or works as a self-employed person (on a trade licence / IČO), whether the data subject is a professional actor or dancer and, if so, the field they studied or pursue, information on abilities, skills or professional competencies that may be used in acting or modelling work (sports activities, playing musical instruments, singing, etc.), height, body measurements (chest–waist–hips), clothing size (S, M, L, etc.), shoe size, eye colour, hair colour, information on previous acting and modelling experience, minimum daily fee requirement, willingness to work as a host/hostess, willingness to perform as an extra (background artist), willingness to appear in underwear or nude, information on tattoos, native language, knowledge of English, knowledge of other foreign languages, nationality, whether the data subject is already represented by another modelling or casting agency, photographs of the data subject, video recordings of the data subject (including audio recordings), ethnic origin, and any physical disability; in the case of children, also information about the legal guardian, i.e. first name, surname, date of birth, address of permanent residence, telephone number and e-mail address.

On the basis of this consent, 9. VLNA CASTING is entitled to provide my personal and sensitive data, for the purposes stated above, also to selected companies and business partners of 9. VLNA CASTING.

Information on the Processing of Personal Data

9. VLNA CASTING, as the data controller, hereby provides information on the manner and scope of the processing of personal data, including the scope of the data subject's rights related to the processing of their sensitive and personal data by 9. VLNA CASTING.

10. VLNA CASTING processes personal data in accordance with applicable law.

Purpose and Scope of the Processing of Personal Data

9. VLNA CASTING processes only accurate personal and sensitive data obtained in accordance with the PDPA, and collects and processes such personal data solely for the stated purposes, to the extent set out below, for a period of 30 years.

Basic purposes of the processing of personal data:

• performance of a contract and provision of services;
• accounting and tax purposes;
• compliance with legal obligations;
• identifying my relevant needs as a customer of 9. VLNA CASTING;
• offering and providing suitable offers to conclude a contract for appearing in audiovisual works, or arranging similar work for the data subject;
• keeping customer records.

Scope of the Processing of Personal Data

9. VLNA CASTING processes personal data to the following extent:

• identification data: academic title, first name, surname, birth surname, date of birth, identity card number; in the case of children, also information about the legal guardian, i.e. first name, surname, date of birth, address of permanent residence, telephone number and e-mail address.
• address and contact data: address of permanent or, where applicable, temporary residence, delivery or other contact address, telephone numbers — mobile, home and work, plus an alternative telephone number (the number and name of a close person), other contact details — Skype, Facebook, Instagram and other social networks, personal website, e-mail address.
• other personal data: bank account number, gender, occupation, possession of a driving licence (and the category of driving entitlement), whether the individual is a VAT payer and/or works as a self-employed person (on a trade licence / IČO), whether the data subject is a professional actor or dancer and, if so, the field they studied or pursue, information on abilities, skills or professional competencies that may be used in acting or modelling work (sports activities, playing musical instruments, singing, etc.), height, body measurements (chest–waist–hips), clothing size (S, M, L, etc.), shoe size, eye colour, hair colour, information on previous acting and modelling experience, minimum daily fee requirement, willingness to work as a host/hostess, willingness to perform as an extra (background artist), willingness to appear in underwear or nude, information on tattoos, native language, knowledge of English, knowledge of other foreign languages, nationality, whether the data subject is already represented by another modelling or casting agency, photographs of the data subject, video recordings of the data subject (including audio recordings), ethnic origin, and any physical disability.

Sources of Personal Data

9. VLNA CASTING obtains personal data primarily from the data subject in the course of negotiations to conclude a contract.

10. VLNA CASTING always informs data subjects when the provision of personal data is necessary for the provision of a specific service, and when it is instead voluntary — although providing such personal data facilitates mutual communication between the data subject and 9. VLNA CASTING.

Processors and Recipients

In order to fulfil the purposes described above, personal data may be processed — in addition to 9. VLNA CASTING and its employees — by processors, on the basis of personal data processing agreements concluded between the processor and 9. VLNA CASTING, as the controller, in accordance with the PDPA.

Nature of the Processing

9. VLNA CASTING processes personal data both manually and by automated means. The protection of personal data is technically and organisationally secured by 9. VLNA CASTING in accordance with the PDPA. 9. VLNA CASTING requires the same level of security from any processors of personal data.

10. VLNA CASTING advises that personal data may, on the basis of a lawful request, be disclosed to third parties vested with the legal authority to require the disclosure of the personal data concerned.

Rights of Data Subjects

The data subject is entitled to request information about the processing of their personal and sensitive data — about the purpose of the processing, the scope or, where applicable, the categories of personal data being processed, the sources of the personal data, the nature of any automated processing, the processors, and the recipients or, where applicable, the categories of recipients of the personal data.

9. VLNA CASTING will provide the requested information without undue delay, for a reasonable fee not exceeding the costs necessary to provide such information.

If the data subject finds or believes that 9. VLNA CASTING, or a contractual processor of 9. VLNA CASTING, is processing personal or sensitive data in a manner contrary to the protection of the data subject's private and personal life, or contrary to the law, the data subject is entitled to:

• request an explanation from 9. VLNA CASTING or its contractual processor;
• request that 9. VLNA CASTING or its contractual processor remedy the situation thus created; in particular, the data subject is entitled to request the blocking, rectification, completion or destruction of the personal data.

9. VLNA CASTING will always inform the data subject, without undue delay, of how their request has been handled. The data subject has the right to lodge a complaint at any time with the Office for Personal Data Protection (Úřad pro ochranu osobních údajů).

I grant this consent voluntarily and I am aware that I may withdraw it at any time.

At the same time, I acknowledge that failure to grant, or withdrawal of, this consent may result in the impossibility of further cooperation or of concluding the anticipated contracts, as such cooperation is based on the sharing of personal and sensitive data for the purpose of finding and/or arranging work for me.

This consent may be withdrawn at any time by sending a withdrawal notice to info@9vlna.cz.
